Managing early peri-implantitis: What we learned

Case study of a patient who developed soft tissue inflammation 6 months post-placement. What we did, what worked, and what we'd do differently. **Timeline:** - Month 6: Patient reported bleeding on brushing around #19 implant - Initial BOP: 4 sites positive - Bone loss: 1.5mm from baseline **What we tried:** 1. Mechanical debridement + CHX irrigation 2. Modified oral hygiene protocol 3. 3-month recall shortened to 6-week **Outcome:** Stabilized at 12 months, no further bone loss. **What I'd change:** Earlier intervention—should have flagged the mild bleeding at month 4 recall.

Excellent case documentation and management of the complication. Non-surgical approach was appropriate for early peri-implantitis. The bruxism diagnosis and occlusal guard likely prevented recurrence.

Managing complications requires skill and patience. Your systematic approach - debridement, irrigation, home care education, and addressing bruxism - shows thorough clinical thinking.

Complete resolution is excellent outcome for peri-implantitis. The 6-month follow-up data showing bone stability is reassuring. Would love to see 1-year follow-up.
